How to Treat Hyperpigmentation: A Clinical Guide

A clinical, step-by-step guide to fading dark spots and hyperpigmentation — from PIH and melasma to sun damage — with the professional ingredients and protocols that actually work.

What hyperpigmentation actually is

Hyperpigmentation is the visible result of excess melanin produced by overactive melanocytes. It is not one condition but a family of disorders triggered by UV exposure, hormones, inflammation, friction or genetics. Treating it effectively starts with naming the right type — the wrong protocol can darken the patch instead of fading it.

The four patterns you will see in the mirror

  • Post-inflammatory hyperpigmentation (PIH) — flat brown to grey marks left behind after acne, ingrown hairs, eczema, a scratch or an aggressive peel. Most common in Fitzpatrick III–VI. Usually fades within 6–12 months with the right routine.
  • Melasma — symmetrical, map-shaped patches on the forehead, cheeks, upper lip and jaw. Driven by hormones (pregnancy, the pill, HRT) and worsened by even small amounts of UV and visible light. Chronic, relapsing, and the toughest type to treat.
  • Solar lentigines (sun spots / age spots) — sharply defined brown spots on areas with chronic sun exposure: temples, cheekbones, the back of the hands, décolleté. Caused by cumulative UV damage.
  • Freckles (ephelides) — small, light tan spots that intensify in summer and fade in winter. Genetic; rarely require medical treatment.

Why most home remedies fail

Lemon juice, baking soda and undiluted essential oils strip the barrier and trigger more PIH. Single-ingredient brighteners (just vitamin C, just niacinamide) plateau after a few weeks because melanin synthesis has multiple checkpoints. A clinical result needs a layered protocol — tyrosinase inhibitors, exfoliation, cell-turnover support and, above all, SPF.

The 12-week protocol that works

Weeks 1–4 — Prep and barrier repair

Switch to a non-stripping cleanser, add a niacinamide 5–10 % serum and a barrier moisturiser with ceramides or centella. Begin daily broad-spectrum SPF 50 applied as the last morning step and reapplied every two hours of outdoor exposure. Without this foundation, the rest of the protocol will rebound.

Weeks 3–6 — Introduce brightening actives

Build up a vitamin C serum in the morning and a tyrosinase inhibitor blend at night (tranexamic acid, alpha-arbutin, kojic acid, azelaic acid). Start a retinoid two evenings per week and increase to four as tolerated. Expect mild flaking — never push past stinging or visible redness.

Weeks 6–10 — Add a professional peel or mask

For PIH, sun spots and superficial melasma, the most evidence-backed protocols are Mesoestetic Cosmelan and the IMAGE Skincare Iluma line. For deep, hormonal or recalcitrant melasma in Fitzpatrick IV–VI, Mesoestetic Dermamelan is the medical reference. Weeks 10–12 — Consolidate and prevent rebound

Continue the maintenance cream, the morning antioxidant and SPF 50 daily — including in winter and indoors near windows. Visible light from screens and overhead lighting can re-trigger melasma; an iron-oxide tinted SPF protects against this.

The active ingredients that actually move pigment

  • Tranexamic acid — inhibits the plasminogen pathway that drives melasma. Tolerated by sensitive skin.
  • Azelaic acid 10–20 % — tyrosinase inhibitor with anti-inflammatory action. Safe in pregnancy.
  • Alpha-arbutin and kojic acid — gentle tyrosinase inhibitors ideal for sensitive or reactive skin.
  • Vitamin C (L-ascorbic acid 10–20 %) — antioxidant that interrupts melanin synthesis and boosts SPF performance.
  • Retinoids — accelerate cell turnover, push pigment out and improve penetration of brighteners. Not for use in pregnancy.
  • Hydroquinone-class actives (Cosmelan, Dermamelan) — gold standard for stubborn cases under professional supervision only.

What to avoid while treating pigmentation

  • Aggressive scrubs, brushes or DIY peels — every micro-trauma fuels new PIH.
  • Hot showers, saunas, hot yoga and intense workouts during the first two weeks of a clinical peel — heat reactivates melanocytes.
  • Skipping SPF on cloudy days, in the car, or at the desk — UVA and visible light pass through windows.
  • Stacking too many actives at once — pick one brightener + one retinoid, not three of each.

When to see a professional first
